Ahead of what promises to be a very exciting start to the next chapter in your education, we have a few pieces of information that we need to collect from you. Parents/Carers, please sign in to complete your child's details. Some of the fields will already be completed with information from your child's primary school. Please check and update where relevant.

 

This form will take approximately 30 minutes to complete. You will need to have the following information:

- Child's Doctors details (surgery name, number & address)

- Child's medical/allergy information if applicable

- Your contact numbers and emails

- At least 1 additional adult contact number (2 is preferable)

- Your national insurance number or NASS numbers
